These are the standard ("long form") physicals needed for any CDL, class A, B, C, and D drivers who are regulated by the D.O.T.You will receive the Medical Examiners Certificate (heath card) upon successful completion of the physical. This is the form that you will bring to the DMV to keep your license current.

Before an official offer of employment is made to a potential candidate, a Pre-Offer Agility Test may be administered to assess the candidates physical ability as it relates to the demands of the position being considered. Medical history is not collected, rather the test seeks to establish the physical demand level at which the applicant can safely perform.
This test is administered after an official offer of
employment has
been made, but before the candidate officially begins working for the
company. The information gathered through this exam
will
document and help to quantify any pre-existing conditions, and is
invaluable when used as a baseline for comparison should a work injury
ever occur.

All you need is a private room, some chairs and access to a restroom. Urine cups are disposed of in the restroom, no mess to worry about, no gowns to change in to.
