When completing the Examination form, please be as detailed as possible regarding any "Yes" answers. Include ALL prescription medication, doses, prescribing doctors and clinic names. Include onset dates for any health conditions. Bring ANY supporting documentation with you. The new NRCME process requires a lot more documentation.
